A girl at her high school graduation hugging a relative

In high school, IEP teams begin planning for life after graduation. This process, called Transition Planning, adds new components to the IEP at age 14 and again at age 16. The earlier families engage in discussions about how they envision the transition experience, the more opportunities young people will have to participate in experiences that can shape their future. Participants will learn:

  • About the IEP transition components and how to use them
  • How to prepare for new conversations and experiences with your child and their school
  • Actions families and students can take to build a transition plan that puts youth on the path to the future they want after high school.
  • Things that families and students should know, do, and ask in each grade to make the IEP and transition process meaningful
